Custom Cookie Order Form

**ACCEPTING ORDERS IN 2026 FOR JAN & FEB ONLY**

Thank you for your interest in placing an order with me. Please fill out the form below to order your custom cookies (be as detailed as possible). Once the form has been submitted, I will contact you directly to confirm your order, discuss delivery/pick-up and any other personal details. As soon as all of the details have been confirmed, I will send you a final invoice via email. Click here to view cookie pricing.

** 2-dozen order minimum (can be mixed large & mini).

** Email bakemydayamanda@gmail.com with any images that you have for inspiration on the designs and/or any additional requests for your order.

**Your order will not be secured until a minimum of 50% deposit is received.

**All sales are final after your order has been paid in full. Refunds are only applicable to cancellations up to 14 days prior to your pickup date.

Which flavor(s) would you like for your order? (1 flavor per dozen) Required

Each cookie will be packaged individually in clear cellophane and heat-sealed to ensure safety and freshness. This packaging is already included in the price of your order of the standard option. If you prefer a different packaging option, please select from the choices below.
